Community Health RNs
Full time position, to care for our patients in their homes, minimum 6 months experience in an acute care setting preferred
32 hours part-time position; to care for patients in the Sawtelle Family Hospice House.
Part-time position, responsibilities include: Performing assessments on patients who are determined to be high risk in an effort to prevent re-hospitalizations; Performing assessments on wound care patients to determine appropriate treatments which will provide favorable outcomes; Performing palliative care evaluations for patient and their families; Performing Face to Face assessments for hospice patients to determine need for recertification; Attending hospice IDT on a bi weekly basis; Coordinating and presenting in-service programs when indicated; Functioning as a liaison between clinical staff, physicians and community resources as directed; In conjunction with the Executive Director, assisting in the development of programs which will enhance the role of the nurse practitioner.
Qualifications: Current Clinical licensure NP to practice in the Commonwealth of Massachusetts with at least five (5) years clinical experience with two (2) years in community health.
Full time position, responsibilities include providing oversight to clinicians in the planning and delivery of patient care to meet agency mission; monitoring documentation and reimbursement of patient plans of care for appropriateness and accuracy; developing clinical department goals and objectives consistent with the organization’s strategic plan and the agency goals and objectives; assisting with the development and implementation of agency operational and financial goals; working closely with the Clinical Director in the development and growth of interdisciplinary programs, analyzing and recommending staffing; managing clinical staff including but not limited to recruitment and hiring, performance feedback and training programs and support; coordinating and presenting in-service programs; participating in agency’s Performance Improvement program; functioning as liaison between clinical staff, physicians and community resources.
Qualifications: Current RN, PT or OT license to practice in the Commonwealth of Massachusetts with at least five (5) years clinical experience with two (2) years in community health.
Full time or part time benefited position; work with referral sources to provide bridge between inpatient and home care and hospice services. Occasional weekend commitment required.
Full time position, to care for our Hospice patients in their homes, minimum 1 year hospice experience preferred.
RN Weekend Baylors
Every weekend, to visit our patients in their homes; benefitted position, minimum 1 year experience in an acute setting preferred.
Certified Home Health Aide
Per diem or part time, to care for patients in their homes.
Requirements for all positions
Strong communication and collaborative skills with a demonstrated ability to speak, write, and communicate in English; Must demonstrate the ability to interact in a positive and courteous manner with patients, families, employees, and affiliations; Detail orientated and the ability to work both independently and as part of a team are essential; Reliable vehicle.
