Services & Specialty Programs
Our all encompassing home care and hospice programs allow you to receive comprehensive health and supportive care for your illness in the comfort of your own home. Registered nurses and rehabilitation therapists handle a variety of cases according to your needs under the direction of your own physician.
Skilled Nursing
Cardiac Disease Management Program (assessment, treatment and patient education for patients with congestive heart failure and other cardio-pulmonary diseases), wound and skin care program, ostomy care and teaching, medication management, IV therapy, post-surgical and medical assessment.
Rehabilitation Therapies
Physical, speech and occupational therapies provide a vast range of in-home care that includes joint replacement, cardiac assessment and teaching, energy conservation and assistance with ADL management.
Telehealth Monitoring System
In home unit monitors the patients' vital signs, weight and oxygen levels; oversight and follow-up provided by a nurse.
Hospice
Hospice provides care that respects a person's need for dignity and comfort during the final stages of life where the focus is not on curing the illness but on providing care and comfort. Care is inter-disciplinary in approach and involves a full range of services including pain control, respite care, guidance, and advocacy. We provide hospice care in the patient's home or at our Sawtelle Family Hospice House (see below) and offer family members special help in dealing with their loss.
Sawtelle Family Hospice House
A home where terminally ill people can receive the same kind of compassionate and comfort care we provide in patients' homes. Our residence preserves the dignity of patients and allows family and friends to continue to play a major role in their loved one's final days without the responsibility of 24-hour duty. Symptom Management and Support Program
Provides careful monitoring and case management for patients with symptoms related to their disease that may or may not be life threatening.
Social Work
Supportive counseling, community resource education, and long term care assistance.
Home Health Aides
Personal care such as bathing and dressing.
Nutritional Management
Assessment and teaching by a registered dietician for cardiac, pulmonary, diabetic and other dietary topics and questions.
Certified Diabetic Nurse
Provides diabetes education and instruction, blood glucose monitoring.
Psychiatric Home Care
Mental health evaluation and medication management.
Alzheimer's Caregiver Program
Designed to support individuals caring for a loved one suffering from Alzheimer's disease and other forms of dementia. Services are free of charge and include home safety evaluations, therapeutic intervention, support and education. This program is supported by a Title III grant from Mystic Valley Elder Services as part of the Older Americans Act. Children's Bereavement Program

Bereavement Support Groups
